  • FireFox 4.x
  • Google Chrome 35.x
  • Safari 5.x

All web browsers are regularly updated to improve performance and remove security vulnerabilities. We highly recommended you regularly update your web browser to the latest version. We also recommend that you clean your browser's temporary Internet files (cache). Otherwise in your installed Adobe Reader software you may get the "File could not be found" error message and blank PDF files (forms).


To utilize the full functionality of this website you must have JavaScript and Cookies enabled in your browser and free Adobe Reader installed on your computer. You also may want to run the Compatibility Test. This will open a Test PDF Form in a new window. If you do not see the phrase, "The Compatibility Test was successful." on the test form, then your system configuration does not meet our requirements. You can still use our application to complete your questionnaire.

Your other option is to save your data to a floppy disk or other media and use a different computer where the Acrobat configuration works successfully.
